The form must be accurately completed and filed at registration. There will be a $30 entry fee if you register before June 15, 2012. If you register after June 15th, the fee will be $40 per individual. There were 760 participants at the 2011 Midwest Nationals Tournament! |

| Awards: Awards will be given to the top 6 individuals in each weight class. Team trophies will be given to the top team in each class. | |
| Uniforms: Must meet National Federation Rules. Headgear is required. | |
| Medical Information: Athletes must be prepared and must submit to a Skin Disease screening at weigh-in. The Chief Medical Officer’s decision is final and not subject to appeal. | |
| Weigh-Ins: All athletes will be required to weigh-in once on Friday, July 6th from 5 p.m. - 8 p.m. at the Illinois Wesleyan University Shirk Center. | |
